You can use one of the following forms to register for the Spring 2001 Gathering. If you are currently a Senior Associate, fill out only the Gathering Registration Section. If you are not yet a Senior Associate, then fill out both the Senior Associate Sign-Up Section and the Gathering Registration Section.
Senior Associate membership donations and registration fees may be made by VISA, MasterCard, check or international money order valid in the U.S (AMEX not accepted). Make checks payable to "Foresight Institute"; checks and bank drafts must be in U.S. dollars drawn on a U.S. Bank. Donations with payment by credit card may be faxed.
On-line registration closed April 16th. To register after April 16, use the print and fax registration form or phone 650-917-1122.
Hotel
Room Rate (Single/Double): $109/night. Mention Foresight Institute to obtain the group rate of $109. Hotel reservations should be made before March 29 to obtain the special rate.
